Long Term Conditions
Managing a long-term condition such as diabetes, cardiac, respiratory and musculoskeletal conditions, chronic pain and fatigue syndrome, IBS, and medically unexplained symptoms, can lead to feelings of low mood, stress, and anxiety. This can make it more difficult to manage a long-term physical health condition. We aim to help you to take control over your physical health by supporting your mental health and helping you make adaptations to better manage your long-term condition.
Support available to you:
We offer a variety of specialised support for people experiencing depression and/or anxiety and are living with a long-term condition.
- SilverCloud: an online Cognitive Behavioural Therapy program
- Virtual workshops
- ‘Living better with long term conditions’ 10-week guided self help therapy group
- 1:1 Guided self help
- 1:1 Cognitive Behavioural Therapy
- 1:1 Counselling for depression

How to access this support:
You must be aged 16 or over and have symptoms suggestive of a mild to moderate anxiety and/or depression. You can refer online through our website where you can complete an online referral form or alternatively please contact our service on 01226 644 900. Your GP or health professional can also refer you to our service.
